What to Include with Your Referral:

• Patient demographics: name, DOB, contact information, insurance (ID#)

• Referring clinician's name and contact information (address, phone, Fax, Email)

• Reason for referral

• Past Medical and Surgical history

• Current medications and allergies

• Relevant imaging — radiology report and imaging CD 

• Pertinent notes, including consultation, procedures/operative reports

• Medical urgency (red flags) ​
