Step 1

Send a referral

If your insurance does not allow you to see a specialist directly, you should talk to your family physician or your primary provider, to ask a referral to be sent to Dr. Rui Zhang @ EliteMD Pain and Spine Clinic, along with the reason for the referral, your insurance and medical information (including your medication list and the related imaging of your pain, such as cervical or lumbar spine MRI/CT, X-ray). If your primary provider has any question, please provide this website information to them or have them contact us. ​

If your insurance does not require a referral to see a specialist, you can directly request a consultation(link).

Step 2

Book a consultation

Click the above button to book an appointment for a new patient consultation. It will take you a webpage to allow you to put in relevant information such as your personal, insurance, medical information and payment authorization, as well as consent forms for HIPPA (privacy), clinic policy, etc.

Once our clinic reviews the information, you will be contacted by a staff member, who will confirm a time/date for your appointment, or gather further information before scheduling an appointment.

After your initial consultation

If you are scheduled for a procedure or an injection after your initial consultation.....You will usually see Dr. Zhang 2-3 weeks after your procedure. If....

Many insurances require a pre-authorization before approval for an injection/procedure. ....

A detailed consultation note will be sent to your referral physician or attorney. If any questions, Dr. Zhang can be reached at .....
